Do you accept health insurance?

Yes! We are a participating provider with most of the major commercial insurance plans in the Florida area. To see a list of insurance plans in which we participate, please visit our insurance page.

Can you share information about my treatment with my primary care doctor?

Yes! If requested, we can forward a copy of your treatment record to your primary care or specialty physician.

Do you provide any radiology services (X-rays, ultrasounds, or MRIs)?

We have Ultrasound at this time.

Can I be seen without an appointment?

Yes! Appointments are not necessary at our sites.

What are your hours?

We’re open weekends and extended hours to better serve you! Please visit our locations page to see the operating hours for each site.

Is treatment available for children?

Several of our doctors treat children over the age of 2. Please give us a call to see which locations have doctors that can treat your child.

What do I need to bring?

Please bring a picture ID and your insurance card, if you will be using insurance.

What if I need to be admitted to Hospital?

Go to nearest Hospital ER, you will be evaluated in the Emergency Room. If needed, you will be admitted under the care of Hospitalist that is Hospital based Doctor. Upon discharge, we will get your hospital records and resume your care in our office.

What can I expect when I come to your clinic?

We have a state of the art clinic with an entirely electronic medical record. Our clinic is new and designed to provide a relaxing and comfortable atmosphere. For your first visit we ask that you fill out a demographics sheet as well as provide us with billing information. You can speed up the process by downloading the forms and bringing them in already filled out.
